There may be occasions when the general media may be interested in your story or interviewing you about your diabetes. Further down the form, you are given the chance to say whether you would be happy to discuss this with the wider media.

No information will be passed on to the media without your permission, and we would always contact you first before giving your details to the media.

Sometimes journalists are looking for people with diabetes from specific ethnic backgrounds. Using the following classifications, how would you describe your ethnic origin.

We would like to hear about any aspect of living with diabetes, volunteering, fundraising or caring with someone with diabetes. We would particularly like to hear from people form different cultural backgrounds, parents of children with diabetes, pregnant women and people who are newly diagnosed, or have been living with the condition for years.
