We’re glad you’re interested in being a patient representative on the National Diabetes Audit.  Being a part of the National Diabetes Audit is a great way to help shape diabetes care all over England and Wales.  

Before you apply, make sure you have read about the role and what is involved to help you answer the questions below.  

Interviews will be held in March.  We will delete your data after the interviews have finished, and we will not use the information you provide for any other purpose.

We ask you to tell us personal information about your diabetes so that we can make sure the role is right for you. We promise to store it securely for up to three months and keep it confidential.  Without your agreement, we are unable to act on your request to apply for the National Diabetes Audit advisory groups.
