We’re glad you’re interested in applying for the National Diabetes Audit Quality Improvement Collaborative (NDA QIC) to improve the care for people with diabetes in England and Wales. Before you apply, make sure you have read the collaborative summary of what is on offer and what is involved.

Your application needs to be submitted using this form by the end of Wednesday 21st April 2021.
